Long Term Care

For those unable to live independently at home we offer medically supervised long term care. In these circumstances we honor the opportunity to help residents and families create a home right here at Washington Center. Our Restorative Nursing team works closely with the rehabilitation department to design individual programs for walking, strengthening, range of motion and similar services. From decorating the resident's room with special keepsakes to designing activities of special interest our staff works to ensure "Home is where the Heart is" for every resident.

Managed Care

Washington Center is a contracted provider with most managed care organizations and is now a member of the Evercare network.

Evercare

Evercare is a national organization whose goal is to provide improved health outcomes for the elderly, disabled and chronically ill.

How do Washington Center patients and residents benefit? Evercare nurse practitioners and case managers coordinate care so that treatments and prescriptions work together for each patient and resident. This unique approach has proven to result in fewer emergency treatments and hospitalizations. Because each situation is unique Evercare offers a variety of plan options. Elderplace

We're also pleased to be part of the Providence ElderPlace family. This is an innovative program of health care and social services for older adults. Their service model is designed to keep older adults as healthy as possible in the community by providing comprehensive health care and social services. Physician Coverage

Residents and patients can choose one of our attending physicians who have admitting privileges in one or more of the area's 14 acute care hospitals or their own doctor if he/she agrees to the regulations for physician oversight in skilled nursing facilities.
